There are 2 self assessments listed below - 'Task Based' and 'Personal'. We recommend that you use them both, or something similar, to ensure that you get a direct payments package that fully meets your requirements.
To ensure that you get allocated the correct number of hours for your direct payments package when the Care Manager comes to visit for your assessment, we suggest that you list the tasks that you do and/or need help with on a daily/weekly/monthly basis.

Remember that there are some tasks that you may only do as little as once or twice per month. It is also worth recording these as you will need them to be included in your direct payments package as well.
This assessment may help you decide how you could best use direct payments, as well as giving you a clearer picture of your needs, ambitions and achievements that you can build on.
